CONFIDENTIALITY AND PRIVACY

  • I am committed to maintaining your confidentiality and privacy.
  • I strictly conform to the HIPAA laws.
  • I will not discuss your case with anyone without your consent with exceptions to legal limits of confidentiality.

FEES AND PAYMENTS

My professional fees are $375 for the initial 60 minute intake appointment and $165, $250, or $330 for ongoing sessions based on time and modality of treatment. Payment is due one business day prior to your scheduled appointment and can be made online via www.Stripe.com.  Patients seen for weekly psychotherapy with or without medication management will receive monthly bills and invoices: the payment is expected within a week. Any collateral work carried out  on your behalf such as filling out forms, writing reports,  making calls,  legal advocacy and similar will be discussed with you prior to commencing such work except in medical emergencies.  It will be charged in time increments based on my usual professional rate of $330 per hour. I do not charge for brief administrative calls but reserve the right to do so for clinical and/or prolonged calls and correspondence.

YOUR INSURANCE

I do not contract with insurance companies. This decision is based on my commitment to providing the best possible medical care while minimizing the impact of insurance restrictions. I realize you have a choice of mental health providers and apologize for any inconvenience. Your health insurance policy may have out-of-network benefits which will allow you to see me. I encourage all my patients to contact their insurance to discuss their OON benefits, as plans may reimburse a significant percentage of treatment costs. Refer to these CPT codes when discussing your benefits with your insurance: 99204 or 99205; 99213, 99214, or 99215; and 90833, 90836, 90838, if therapy is part of the treatment. An itemized billing statement in the required Superbill format is provided monthly. Flexible Spending Account can also help pay for your visits. FSAs and HSAs are offered by many employers; please check with your benefits department for details.

CANCELLATIONS

48 hours advance notice by email is needed if you would like to cancel or reschedule an appointment.  You will be charged for the full cost of a session if 48 hours notice is not given or if you fail to show up for a scheduled session. Exceptions are official snow emergencies and safety warnings.
